Questions

Does a fragile stamp mean the carrier has to be careful?

Not by itself. Carrier liability is set by the contract of carriage and the packaging standard, not by what is printed on the box. The mark is an instruction to whoever is handling it, which is useful, but it is not a guarantee and should not replace proper packing.

What size should a shipping mark be?

Big enough to read at the distance somebody decides how to pick the box up. Set the physical size before downloading so the file carries the dimensions, rather than being scaled by whatever prints it.

Should it be red?

Red is the widely understood convention for fragile and it costs nothing to follow. Where a carrier or customer specifies their own marking scheme, follow theirs.
