A QR code in a stamp is a good idea that fails on a detail: the code needs a clear margin around it and enough physical size to be resolved by a camera, and a stamp is small. Get those two things right and a stamped document can carry a link to a verification page, a menu, a payment page or a record. Get them wrong and you have printed a grey square that nobody can scan and everybody assumes is broken.

Questions
How small can a QR code be and still scan?
About 20mm square in practice for a phone at arm length, and more if the code holds a long URL. Shorten the link before you generate the code: fewer characters means larger squares means a code that scans from further away.
Why will my QR stamp not scan?
Usually one of three things: no quiet zone, because text or a border was placed against the code; too small, because the stamp was sized for a signature block; or too little contrast, because the code was drawn in the stamp colour instead of black.
What should the QR code point at?
Something short and stable. A verification page, a document record, a payment link or a menu. Avoid pointing it at anything that will be moved next year, because the stamped documents cannot be updated.
Can the QR code carry the document number?
Yes, if the link includes it. That is what makes each stamped document individually traceable rather than all pointing at the same page.
