That is the common form, and a certified true copy sample stamp looks like the block above: the words, who certified it, their capacity to do so, a signature and a date. Who may certify is the part that varies and the part that matters, and it is set by whoever will receive the copy. Banks, universities, immigration authorities and courts each publish their own list, and several will only accept a lawyer, a notary, a commissioner for oaths or a named official. Check the requirement with the receiving body before certifying anything, and only certify a copy you have compared against the original yourself.

Questions
What has to appear on a certified true copy stamp?
Usually the words "certified true copy", a signature line, the certifier’s name or title, and a date. Some jurisdictions want the capacity in which the person certifies, and some want a reference to the original. Check what the receiving body asks for, because it varies.
Who is allowed to certify a copy?
That depends entirely on where you are and what the document is. Commonly a notary, a commissioner for oaths, a solicitor, an accountant or a bank official. This tool draws the stamp; it does not make you a certifier.
Why does the date fill itself in?
Because the certification is about a check performed on a specific day. A stamp carrying the day it was designed says the comparison happened months before the copy existed.
