What are the four submittal review actions?
The set most offices use is: no exceptions taken, make corrections noted, revise and resubmit, and rejected. Some replace the first with approved and the second with approved as noted, which means the same thing in ordinary language and less in a contract. Use whatever wording your specification section actually names, because that is the wording the contractor is entitled to rely on.
Why does the stamp need a disclaimer?
Because reviewing a submittal is a narrower act than approving the work. The standard wording says the review is for conformance with the design intent of the contract documents only, and that it does not relieve the contractor of responsibility for dimensions, quantities, fabrication, means and methods, or coordination with other trades. Leave it off and the block implies more than the reviewer meant.

Is a digital submittal stamp acceptable?
That is a question for the specification and, where the reviewer is a licensed professional sealing the work, for the licensing board in that state or province. Many now accept and some require electronic submittals. Rules on applying a professional seal digitally vary and are set by the board, not by the software, so check the requirement rather than assuming the file settles it.
