Forty signatures before lunch, and under every one of them the same four lines: name, qualification, designation, registration number. A doctor signature stamp exists because writing that out by hand forty times a day is not a use of a clinician.

It is also the mark a pharmacist, an insurer or a hospital records office will look for when they need to know who signed. A prescription with an illegible signature and no printed name is a prescription somebody has to telephone about.
What goes on a doctor signature stamp
The convention is a four line block, and it is a convention because each line answers a question somebody downstream actually asks.
The name, with the title. Dr. Priya Nair, not P. Nair. The person reading it has to be able to look you up.
The qualification. MBBS, MD, BDS, MRCP, whatever you hold. This is what tells a pharmacist the prescription is within scope.
The designation and the practice. Consultant Physician, and the name of the clinic, hospital or medical practice. A stamp that names a person but not a place leaves the reader without a route back.
The registration number. The line that matters most and the one most often left off. It is what a council register is searched by, and it is the difference between a mark somebody can verify and a mark they have to trust.
Most add a fifth line for the city, and many add a signature line so the stamp sits under a real signature rather than pretending to be one.
The rest of the set a practice presses
A physician signature stamp is the personal one, and it belongs to one clinician. It should never be shared, and a locked file is a better custody arrangement than a rubber block in an unlocked drawer.
A clinic stamp or medical practice stamp is the round one carrying the practice name and registration, and it goes on letters, referrals, certificates and the patient file copies a records office keeps.
A prescription stamp is the compact block designed to fit the space a prescription pad leaves, which is smaller than people expect.
A dental stamp carries BDS and the practice, and a dental clinic uses the same four line logic with different letters after the name.
A pharmacy stamp goes on dispensing records and on the copies a pharmacy keeps.
A received stamp with a date for the records office, because a clinic receives more paper than it sends and the day something arrived is what an audit asks about.
Some practices add an allergy warning stamp for notes, and a red one for anything urgent. These are conventions of the practice rather than requirements, and they are worth agreeing once rather than improvising per clinician.

What this tool will not decide for you
Whether a stamped signature is acceptable on a prescription, a fitness certificate, a death certificate or an insurance form is set by the council or board you are registered with, and it differs sharply by country and by document. Several regulators are explicit that certain documents need a wet signature and some accept an applied one on routine correspondence. Check your own registration rules rather than anything written here.
This is a design tool. It produces the mark, not the authority behind it. Make one only for credentials you actually hold: a stamp naming a registration that is not yours is forgery, and the terms of use prohibit it.
Size, colour and the practical bits
Round clinic seals are usually 25mm to 30mm across; a rectangular signature block is usually around 55mm by 22mm, which is the size that fits a prescription without covering the drug. Blue is the most common ink for medical stamps and black is the safest for anything that will be photocopied repeatedly. Keep the type large enough to survive a fax, because parts of healthcare still run on one.

Can a clinic have one stamp for everybody?
The practice stamp, yes: it names the clinic and every clinician can use it. The signature stamp, no. That one identifies a person and should belong to that person alone.
What ink colour do medical stamps use?
Blue is the most common and reads as clearly distinct from black body text. Black survives repeated photocopying best. Red is worth keeping for the urgent and allergy marks, so that colour means something on a set of notes rather than being decoration.
